Home Team Forms

Volleyball Packet to be completed and handed in to the VHS Athletic Department ASAP Students enrolled at Venice High School who wish to participate in a FHSAA sanctioned sport for the current school year, must have on file in the Athletic Department the following forms: current athletic physical, Notarized Parent Release & Hold Harmless consent form, FHSAA (EL3) medical release form, HIPPA form and a Standards for Participation form. All participation forms are good for 365 days; therefore a new set has to be on file each school year.  The Florida High School Athletic Association (FHSAA) eligibility requirements are a 2.0 grade point average.
